General Office Policies

• Please arrive 15-20 minutes prior to your initial appointment for requisite paperwork.
  
  
• Please bring insurance cards and referral authorizations, if necessary.
  
  
• Co-payments are collected on the day of service.
  
  
• Cash checks or major credit cards are accepted.
  
  
• We validate for 30 minutes of parking in the lot adjacent to the medical building.
