• Please arrive 15-20 minutes prior to your initial appointment for requisite paperwork.
  
  
• Please bring insurance cards and referral authorizations, if necessary.
  
  
• Co-payments are collected on the day of service.
  
  
• Cash checks or major credit cards are accepted.
  
  

Parking is available at:

Corner of El Molino and Union - Charge is $1.00 per hour or $5.00 max

Medical Building at the corner of Madison and Union - Charge is $6.00

Parking structure next to Fuller Seminary on Union St. and Oakland Ave - Charge is $6.00

Street parking is also available at no charge on Union - 1 hour parking and Madison - 2 hour parking
